Tijdens een reis naar Vietnam wordt de vooraanstaande Mexicaanse dokter Robert Smith aangevallen door een groep terroristen. Wanneer hij weer thuis is ontdekt hij dat hij een ongeneeslijke ziekte heeft opgelopen. Vanwege het trauma van de aanval en de ziekte belandt hij in een neerwaartse spiraal van gekte en verandert in een koelbloedige seriemoordenaar.
