Een mysterieuze man ontwaakt in een mortuarium. Hij begeeft zich naar de psychiatrische afdeling waar hij in contact komt met de dokter Daniel Forrester, die meteen een interesse in de John Doe toont. De man beweert beheerst te worden door een zwartheid die in hem woedt. Bij verder onderzoek komen duistere geheimen aan het licht omtrent de man, die een gruwelijke waarheid onthullen.
