Leon S. Kennedy, een Amerikaanse geheim agent, glipt de 'Oost Slavische Republiek' binnen om geruchten te verifiëren dat Bio-Organische Wapens (BOWs) worden gebruikt tijdens de lopende burgeroorlog in dat land. De Verenigde Staten en Rusland willen gezamenlijk het land binnenvallen om een einde te maken aan de oorlog, en de Amerikaanse overheid beveelt Leon om het land onmiddellijk te verlaten. Hij wil echter de waarheid achterhalen en negeert de bevelen. Hij begeeft zich naar het oorlogsgebied om een einde te maken aan de tragedies veroorzaakt door de BOWs.
