Het verhaal van Lewis Shaler (Dougray Scott), een overwerkte arts en toegewijde, alleenstaande vader op weg naar huis met zijn zoontje in de laatste trein vanuit Londen, waar hij een flirterige vreemdeling ontmoet. Het leven lijkt voor Lewis eindelijk weer even echt zin te hebben. Maar dan slaat het noodlot toe. De machinist lijkt plots verdwenen en de remmen zijn gesaboteerd. Een wraakzuchtige sociopaat heeft controle over de trein genomen en is vastbesloten deze te laten crashen en alle passagiers mee te nemen naar het graf...
