Hermes Papauran is een vooraanstaand politieagent die betrokken is bij de wrede war on drugs van de Filipijnse overheid. Verteerd door schuldgevoelens over de in scène gezette bloedbaden waarvan hij getuige is, krijgt hij een slopende huidaandoening en vlucht naar het platteland. Supremo Macabantay, een louche figuur uit Hermes' verleden die net is vrijgekomen, zint op wraak op de herstellende politieman.
