Sarah (Shauna Macdonald) weet te ontsnappen uit de verschrikkelijke grot waar de gebeurtenissen uit het eerste deel plaatsvonden. Een toevallige passant voert haar voor verzorging naar het dichtstbijzijnde ziekenhuis. Een lokaal reddingsteam brengt haar terug naar het grottenstelsel waar ze uit wist te ontsnappen in de hoop om met haar hulp een paar overlevenden te vinden. Eenmaal aangekomen worden ze geconfronteerd met dezelfde nachtmerrie als voorheen.
