Na de dood van seriemoordenaar Jigsaw zet forensisch detective Hoffman zijn werk voort. FBI-agent Strahm, die ternauwernood een test van Jigsaw heeft overleeft, ruikt onraad en duikt in het verleden van Hoffman. Ondertussen leren vijf op het oog willekeurige vreemdelingen van Jigsaw een gruwelijke les over samenwerken.
