Wanneer een mysterieuze brief James terugroept naar Silent Hill, op zoek naar zijn ware liefde, treft hij een ooit zo herkenbaar stadje aan, getransformeerd door een onbekend kwaad. Terwijl James dieper de duisternis in afdaalt, ontmoet hij angstaanjagende figuren, zowel bekend als nieuw, en begint hij te twijfelen aan zijn eigen geestelijke gezondheid, terwijl hij worstelt om de realiteit te begrijpen en lang genoeg stand te houden om zijn verloren liefde te redden.
