Het is bijna drie decennia geleden dat het Rage-virus ontsnapte uit een laboratorium voor biologische wapens, en er is nog steeds een strikt voorgeschreven quarantaine van kracht. Sommige mensen hebben manieren gevonden om te midden van de geïnfecteerden te bestaan. Een van die groepen overlevenden woont op een klein eiland dat met het vasteland is verbonden door een enkele, zwaar verdedigde verhoogde weg. Wanneer een van de bewoners het eiland verlaat om het donkere binnenland van het vasteland binnen te dringen, ontdekt hij geheimen, wonderen en verschrikkingen en dat niet alleen de geïnfecteerden, maar ook de overlevenden zijn gemuteerd.
